Community Health Improvement Plan

The El Paso County Community Health Improvement Plan (CHIP) is a strategic, community-driven effort aimed at addressing the priority health issues identified in the Community Health Assessment (CHA). The CHIP outlines a long-term plan to improve community health by establishing goals, measurable objectives, and actionable strategies for 2025-2029. Developed in partnership with the Healthy Community Collaborative (HCC)—a diverse group of community members representing community health systems, hospitals, non-profit organizations, city and county government and interested citizens—the 2025-2029 CHIP focuses on the four leading health issues areas: barriers to accessing health care, suicide prevention, housing stability, and drug overdose. This collective approach fosters shared responsibility and aligns with state and national public health frameworks. With support from El Paso County Public Health, the HCC is working to implement the CHIP to create lasting, equitable improvements in health for all residents.
