Effective Oct. 15, 2024, grandparents will no longer be eligible to request birth certificates or death records for grandchildren based on their relationship. Eligibility to apply will be established on legal grounds and acceptable documentation, such as but not limited to a certified court order of custody, will, insurance/benefits, bank statements, property deeds, etc.

El Paso County Public Health Vital Records provides certified copies of birth and death records for vital events that occurred in Colorado. Please follow our handy guide below to access a birth or death record.

Birth certificate

To receive a standard birth certificate, you must submit the following:

  1. Complete application | Español
  2. Copy of your identification and proof of eligibility. Birth certificate eligibility requirements can be found through the Colorado Department of Public Health & Environment (CDPHE) Vital Records page or by clicking on this link.
  3. Non-refundable application fees.
  4. Mail to address listed on the application

Death certificate

To receive a copy of a standard death certificate, you must submit the following:

  1. Complete application | Español
  2. Copy of your identification and proof of eligibility. Death certificate eligibility requirements can be found through the Colorado Department of Public Health & Environment (CDPHE) Vital Records page or by clicking on this link.
  3. Non-refundable application fees.
  4. Mail to address listed on the application

In-person

Our office is open to the public by appointment or you are welcome to walk-in. Walk-ins will be served as time permits.

Proof of relationship and a valid photo ID are required to process all requests. Any documents presented for the purpose of establishing identity or relationship must be fully intact and easy to read.

Costs and fees are as follows:

  • First copy of birth certificate: $20; each additional copy of birth certificate (on the same order) $13
  • First copy of death certificate: $20; each additional copy of death certificate (on the same order) $13
